It would be hard to say why it happened, could have been many things, but I suspect that the last person who removed the nuts, didn't tighten them down far enough. I don't remember even touching them when I had Lewy. As for what you will need to do now is replace the axles, bearings and hubs. Front hubs off any EA81 should work.
