I'm sure this has been done before but I just did it today and thought I would share what I did with everyone.
I had the rear pads on my drum brakes shatter into little pieces then I heard about some Subaru XT's being in the local wrecking yard. I went and grabbed all the parts for the disk brakes on the XT (backing plate, caliper, pads, rotor) and swapped them into my 1979 Wagon originally with rear drums.
The backing plates needed to be ground down a bit to clear the larger stamped/welded axle carrier. I then cut a notch into the carrier so that I could access the caliper bolt since you can slide on the rotor unless the caliper is removed.
Pics should be self explanatory.
I guess the pics are not working so I just updated my blog post which has the images.

First pic, directly from the ad.
Will get more once I clean it up and take some of my own.

*Update*
I bought the Wagon, 4 speed 4wd D/R. It hadn't started in over 8 months, and after we got the fuel pump to suck in fuel, the thing instantly started and purred like a subie kitten.
Gonna put a tapered washer in the front axle and drive it home hopefully tomorrow. Almost no rust anywhere, only a few dings, gonna be a great fixer/driver.

way to go man! i like the oregon/washington liscence plate combo.
RV
Yeah get that fixed, A really nice friend of mine got pulled over and she was treated like a criminal because the body shop put on a new bumper and forgot to switch out the plates, so she had an Oregon plate on front and Washington on the rear.
Great build, I am looking to pickup a 70s Leone here in the next few days.

I have been looking for an older project car to build and I think I have found one I want to do. its a late 70s Leone wagon, 78 or 79 4-door. I am going to go look at it but wanted to ask experienced people what I should look for or be aware of?
Its a 4wd, but I don't know which transmission is in it yet. Was planning on making this my daily driver for a while so should I stay away from any particular trans? I believe they came in 4 spd, 5spd, and a 3 spd auto?
tnx
