shifts fine when cold...5 mins after warm up: shifts fine 1st to 2nd...then you have to let off the gas for it to shift into higher gear

any ideas what could cause that?

Start with a fluid flush. Search here and you will see the different ways to do it.

 

Sounds like one of the valves is sticky.

If you can shift it manually and all the gears work, it is sticky valves. Change the fluid.
