Im not entirely sure. I think it overheated at first and then maybe a wire melted? I have a feeling it was due to the fuel lines though. Maybe they were old and cracked in some place.

    yes, you need both pcv ports... as you say.    Fresh air enters on the passenger side and exits from the driver side.    Jesek has a good write up on how to set up your pcv circuit with catch.    Clea
