Shudder from Right front... how do i fix?

Ok, in the RX, when I get off the gas at about 40 the right front wheel seems to shudder, fairly badly.

 

Now, what could be the cause of this? I still haven't gotten an alignment after converting to 5 lugs... could that be it? Or maybe it could be the axle nut? I've already tried the lugs, they were tight as could be.

 

Thoughts?

Bad Axle or Axle nut torqued incorrectly.

 

Brake pads separated and lining is not attached to the backing anymore. Had this happen with my 86 Hatch a long time ago....weirdest thing ever. 4 different places including 2 Chicagoland Subaru dealerships couldnt find it.
